Summary

The 2023 European Society for Cardiology (ESC) guideline for the management of acute coronary syndromes (ACS) included all three subtypes of ACS in one guideline document for the first time. This included ST elevation myocardial infarction (STEMI), non-ST elevation myocardial infarction (NSTEMI) and unstable angina. A novel aspect of this guideline was the introduction of the concept that all three ACS subtypes can be best considered and managed as a clinical spectrum. This chapter aims to highlight the aspects of the 2023 ESC guidelines for the management of ACS that may be most relevant for the interventional cardiologist. This includes sections on invasive strategies, antithrombotic therapy, technical aspects of revascularisation and the management of multivessel disease. Throughout the guideline, there is a particular focus on demonstrating the common principles underpinning the management of patients presenting with ACS. Further detail regarding all topics discussed in this chapter is provided in the main guideline, available on the European Heart Journal website 11.
