Clopidogrel vs aspirin monotherapy in high-risk patients after PCI

SMART-CHOICE 3
Objective
to compare the efficacy and safety of clopidogrel versus aspirin monotherapy in patients who completed a standard duration of dual antiplatelet therapy (DAPT) following PCI with drug-eluting stents
Study
multicentre, open-label, randomised controlled trial at 26 sites in South Korea
Population
patients at high risk of recurrent ischaemic events (prior MI, diabetes, or complex coronary lesions) who completed standard duration of DAPT post PCI
Endpoints
major adverse and cerebrovascular events (MACCE), a composite of all-cause death, myocardial infarction, or stroke at 3-year follow-up
Conclusion
Compared to aspirin monotherapy, clopidogrel monotherapy significantly reduced the cumulative incidence of MACCE at 3-year follow-up without increasing the risk of bleeding.
