12. Percutaneous intervention for aortic valve stenosis

Deferred PCI vs PCI prior to TAVI

PRO-TAVI
Objective
to assess whether deferring percutaneous coronary intervention (PCI) is noninferior to routine pre-procedural PCI in patients with stable coronary artery disease undergoing TAVI
Study
investigator-initiated, multicentre, open-label, non-inferiority randomised controlled trial (margin 11%)
Population
patients with severe aortic stenosis and significant coronary artery disease undergoing TAVI
Endpoints
composite of all-cause mortality, myocardial infarction, stroke, and major bleeding at 1 year
PRO-TAVI
PRO-TAVI
Conclusion
Among TAVI patients with coronary artery disease, deferred PCI was non-inferior to PCI prior to TAVI for the 1-year composite of all-cause mortality, myocardial infarction, stroke, and major bleeding, suggesting that deferral of PCI can be appropriate in a selection of patients.
